Practice Policies & Fee Schedule

NATUROPATHIC MEDICAL SERVICES 

Naturopathic Medicine is truly Integrative Medicine with an emphasis on the prevention, diagnosis, management, and treatment of both acute and chronic health conditions. Dr. Gayl Hyde, Naturopathic Doctor, PC (the “Practice”) uses methods of treatment that are permitted under the Arizona Naturopathic Physicians Medical Board and the California Board of Naturopathic Medicine, which may include but are not limited to nutritional and lifestyle counseling, botanical medicine, homeopathic medicine, nutritional supplements, and prescription drugs, including hormone replacement therapy. 

APPOINTMENT POLICY 

Initial appointments customarily last 60-90 minutes and are conducted via telehealth. Follow-up appointments traditionally last 30-40 minutes. The Practice generally recommends that all new patients schedule a follow-up appointment every 2-3 weeks initially, and subsequent appointments at least once every 3 months, or as recommended/needed. This is especially important if you are being treated for MCAS, thyroid dysfunction, or hormone balancing, or are being treated with a constitutional homeopathic remedy. 

Please avoid discussing other people's health concerns during your visit. Time is limited, and it’s important that you receive the attention and time you need to be best assisted. Health advice, supplement recommendations, and prescriptions cannot be given for non-patients or to third parties, including parents and legal guardians, outside formal consultations for the individual of concern. 

Patient consent must be obtained before third parties, including friends and family, can attend consultations. In the event a third party is speaking for the patient or seeking health advice of their own, that party will be asked to focus on the patient or wait in another room while the consultation is concluded. A patient’s privacy cannot be guaranteed when third parties who are not employees or professional associates of the Practice are present during a consultation. 

There are fees associated with all consultations, including but not limited to follow-up appointments, lab reviews, and phone consults. Fees for follow-up appointments, lab reviews, and other services are not included in the initial consultation fee. Unscheduled calls, e-mail, and text messaging are not to be used in place of an appointment. Please keep all health concern questions for appointments. Medical advice will be discussed only during scheduled consults. 

Research requested by the patient is a billable service and will be charged at the doctor’s hourly rate. This includes, but is not limited to, requests to review unaffiliated websites for opinions and feedback based on the information on those sites. 

CANCELLATION AND LATE ARRIVAL POLICY 

A 24-hour notice is required to avoid a cancellation charge. If you need to reschedule or cancel your appointment, please give the Practice at least 24 hours' notice to avoid cancellation or other applicable fees. If you late-cancel or fail to show up for your appointment, you will be charged $95 for a missed initial consult and $50 for a missed follow-up appointment. 

Late arrivals may result in a shortened consultation/service. To accommodate all clients, late arrivals may result in a shortened consultation. 

PROFESSIONAL FEES 

An initial consultation is customarily billed at $250, unless more than 90 minutes are needed to conclude the appointment. A regular follow-up appointment (45 minutes) is typically billed at $125. Laboratory testing, imaging, medications, supplements, and other professional services are all billed separately. Extended consultations, or more than 90 minutes for an initial appointment or more than 45 minutes for a follow-up appointment, are billed based on the length and complexity of the appointment. 

Laboratory testing, nontraditional tests, or home-use tests may incur additional fees, included in the costs or as separate line items, for lab analysis, processing, venipuncture, specimen collection and/or handling, transport and/or shipping of the specimen, and laboratory test kit(s).

PAYMENT POLICY 

Payment is due in full at the time services are rendered unless other arrangements have previously been made in writing. MasterCard, Visa, American Express, Discover, and debit cards are accepted. Health Savings Account (HSA) cards and Flex Savings Account (FSA) cards may or may not be approved (determined by your card provider). Checks are not accepted. There are no refunds. 

INSURANCE POLICY 

The Practice operates on a cash-only basis; it is not an in-network provider and does not bill insurance. You may not submit receipts to your insurance provider for reimbursement. 

LAB TESTING 

Lab testing is not included in the consultation fee. Any labs that the Practice orders for you will be through a professional co-op in partnership with LabCorp. The cost of the labs is to be paid directly to the Practice. You are responsible for providing a printed-out copy of your lab requisition to the lab when you arrive for your blood draw. 

The Practice reserves the right to change its Practice Policies and Fee Schedule and update this notice. Any revised notice will be posted on this website and available upon request.
