HIPAA & Your Protected Health Information
Dr. Gayl Hyde, Naturopathic Doctor, PC (the “Company”) maintains the privacy of medical and health information of any individual for whom it provides services ("Protected Health Information" or "PHI") and endeavors to comply with all relevant state, national, and international laws and regulations, including the U.S. Health Insurance Portability and Accountability Act (HIPAA). In addition, all personal information is confidential and not disclosed to third parties unless under a court order or upon receipt of signed documentation from the patient authorizing the release of the requested information. This includes but is not limited to name, address, phone number, social security number, and e-mail address. Information regarding a minor's PHI may be disclosed to the legal guardian as required by law. It is important that you understand that your information can be used and shared in the following ways:
For your treatment and care coordination. Multiple health care providers may be involved in your treatment directly and indirectly.
With your family, friends, relatives, or others that you identify as being involved in your health care or health care bills.
To protect the public's health, such as reporting when the flu is in your area or if you are a physical threat to yourself, your doctor, the community, or your family.
To make the required reports to the police, such as gunshot wounds.
Obtain payment from third-party payers.
YOUR RIGHTS REGARDING PHI
You have the following rights with respect to your protected health information (PHI):
Right to Access: You may request to review or receive a copy of your medical records.
Right to Amend: You may request corrections to your PHI if you believe it is inaccurate or incomplete.
Right to Confidential Communications: You may request that the Company communicate with you in a specific way (e.g., at a certain address or phone number).
Right to Restrict Use and Disclosure: You may request limits on how the Company uses or shares your PHI, though it may not always be able to grant such requests if required for treatment, payment, healthcare operations, or by law.
Right to an Accounting of Disclosures: You may request a list of certain disclosures that the Company has made of your PHI.
Right to a Paper Copy: You may request a paper copy of this notice at any time.
THE COMPANY’S LEGAL DUTIES
The Company is required by law to:
Maintain the privacy of your protected health information.
Provide you with this Notice of Privacy Practices.
Notify you in the event of a breach of unsecured PHI.
Follow the terms of this notice as long as it remains in effect.
The Company reserves the right to change its Privacy Policy and update this notice. Any revised notice will be posted on this website and available upon request.
